Our Care Coordination & Family Support services provide an extra layer of personalized support for residents and their loved ones. We help families navigate healthcare and community resources, coordinate appointments and services, facilitate communication with family members and care teams, and offer guidance during transitions and times of change.
Whether a resident needs help staying organized, a family needs support from a distance, or everyone could benefit from a trusted advocate and partner, we’re here to provide compassionate, relationship-centered support that promotes dignity, connection, and quality of life.
Services may include:
* Appointment coordination, reminders, and advocacy
* Communication with family members and care teams (with consent)
* Resource navigation and referrals
* Support during transitions and life changes
* Advocacy and personalized support
* Family meetings and check-ins

Host a Wellness Day and give your residents the gift of relaxation
Rooted Care Collective partners with senior living communities to host on-site Wellness Days for residents. Our licensed massage therapists, trained in arthritis care and pain relief, provide gentle 15-30 minute massage sessions in a comfortable common area. Residents simply sign up for a time slot and enjoy therapeutic care without leaving their home. Wellness Days are an easy way for communities to support resident comfort, relaxation, and overall well-being.
